I think you'll find that there are small variations between printer models, so there are no settings that will work for everyone. That's why the settings are provided instead of it being hard coded.
I would print a sample on blank paper, and hold it up to light with the provided form. Then make adjustments until you get it to look right.
I use a HP Inkjet 110, a very basic printer. Hope someone may be able to help.
It is not the printer, it is the software. You need to go into the program setup and adjust the offsets for your system, we all have had to do it. Paul described his method that worked.
I used the original document and sent it through after each setting change. Took a number of tries.
